MHSAA Physical Form
physical2page.pdfSports Physicals All student-athletes must have a sports physical performed by their physician after April 15 to be eligible to participate in athletics during the current l year. The physical form must be dated and signed by the doctor, parent, and student-athletes. We will accept the MHSAA physical form or your doctor’s sports physical form. The MHSAA physical form is available in our office as well as online on this page. Physical forms must be turned in to the main office. Coaches are not permitted to accept physical forms and students may not practice until the form has been turned in to the office. Please identify yourself as a Wayne-Westland student-athlete when making your appointment.

Wayne Memorial is soliciting new nominations for the Wayne Memorial High School Athletic Hall of Fame. The plaques were previously housed at Jake's, but will be relocated to Alumni Arena on the Wayne Memorial campus. Induction of new members will be held on June 1, 2018. New nominations must be received by March 1, 2018.
